WHAT IS HOME CARE?

Home care is broad range of professional health care and support services provided in people’s homes. Treatment varies from client to client but home care is appropriate when a person can safely stay at home but still needs ongoing care that cannot easily or effectively be provided solely by family and friends. Personal assistance services is most appropriate for those who may need help with activities of daily living (ADLs) or instrumental activities of daily living (IADLs). This includes: bathing, dressing, ambulating or light housekeeping and cooking.

WHO IS ELEGIBLE?

People of all ages with chronic health and acute care needs can receive provider home care services. Provider home care is for persons who need personal assistance in the home through attendants to perform day-to-day tasks such as hygiene, housekeeping, cooking, shopping or even walking steadily without assistance. The payor for these services typically determines the level of care and those who qualify, in most cases medical orders from a physician are required for home health care.

HOW DO I START?

Once agency receives a referral, a staff member will come to your home to talk to you about your needs and ask you some questions about your current situation. Referral is processed with Texas Department of Aging and Disability, HMO’s and a caseworker will evaluate need for services.

PAYMENT ASSISTANCE INFORMATION

The Department of Aging & Disability directs the program. We are currently contracted with the following providers: Regular/Traditional Medicaid, Superior Healthplan, Molina Healthcare, Driscoll Health Plan, and VetAssist.

ASSITANCE QUALIFICATION REQUIREMENTS

Qualifications are decided through Medicaid and HMO’s are based on income, assets, and health conditions that cause functional limitations in performing Activities of Daily Living (ADL’s). Financial eligibility may also be determined by the verification of the applicant’s Medicaid status by the Texas Department of Aging and Disability Services.

HOW OFTEN ARE THE VISITS?

As often as needed. The provider or assistant will provide the necessary care and stops visiting the client when the authorization ends unless its extended by the insurance
